Form LocationsYou can download most Club Sports forms from the links below. Hard copies of blank forms are also available in the Club Sports Office (Room 114 of the Malley Center). -
What to do with Completed Forms?If you are a Club Sports officer, team member, or prospective team member and you have completed forms to turn in, please place them in the Club Sports Inbox.
The inbox has a bright green label and is located on the wall (behind the door) in Room 114 in the Malley Center.Do NOT turn in your completed forms in to any place other than the Club Sports Inbox.
You may NOT leave your Club Sports forms with the Malley Center Front Desk staff.
Emailing Forms:
Forms with an asterisk* are PDFs that can be emailed to the Assistant Director of Recreation at epatchett@scu.edu as long as you have an updated version of Adobe Reader on your computer (does not work with Mac computers).
Make sure you open the file, fill out the form, and save it to your computer PRIOR to sending.
We must receive a hard copy of any form that does NOT have an asterisk.

Forms Required to Join a Club Team- Informed Consent & Release of Liability (updated 8/26/09)

Prospective team members are not eligible to participate with a team in any practices or games until the above 2 forms are turned in, reviewed by the Club Sports Office, and the student's name is listed on the team's official roster. Please check with the team President to find out if you have been listed as okay to participate on the team's official roster. - Informed Consent & Release of Liability (updated 8/26/09)

Travel Related Forms- *Travel (Due 4 days prior to departure)
- Driving Roster (Due prior to driving)
- University's Vehicle Policy (All drivers must read to become approved)

Financial Forms- Advance Request Form (Due 2 weeks prior to need-by date)
- *Business Expense Form (Due within 3 days of returning, use for travel related expenses only)
- *Fundraiser Request (Due 2 weeks prior to event)
- *Non-Travel Reimbursement (Due within 3 days of purchase, use for all non-travel expenses)
- Program Funding Summary (Due 3rd Friday of Fall Quarter)
- *Sponsorship Request (Due prior to finalizing contract)
*Okay to submit via email.
- Advance Request Form (Due 2 weeks prior to need-by date)

- *Guest Speaker/Honorarium (Due 4 weeks prior to payment date)
- *Campus Safety Parking Permit Application (Due 2 weeks prior to need-by date. Permit will be delivered to our office for the coach or team president to pick up.)

- Coach/Instructor Agreement (Required prior to start date) - Updated 9/22/09

Practice and Game Forms- **Assumption of Risk and Liability Waiver (Due immediately following an event hosted inside the Malley Center)
- Facility Request Form (Due 2 months prior to event)
- *Game Schedule Request (Due 2 months prior to event)
- Injury Report (Due within 24 hours)
- *Practice Schedule Request (Due 2 weeks prior to event or as requested)
- *Results (Due within 3 days of returning)
- Stanton Field Agreement Form (Required to be signed by 100% of team players and coaches prior to a team getting access to Stanton)
*Okay to submit via email.

- *Intent to Continue (Due at New Officer Training in Spring Quarter)
- Royalty Waiver Form (Required along with the Apparel Request Form if using school logo on apparel)
