Below are forms you may need to fill out during the financial aid process. Santa Clara University Forms

  • SCU Satisfactory Academic Progress Petition
    This form is to be used by students who are petitioning to maintain their financial aid because they have not met the minimum academic standards or based on other extenuating circumstances.
  • SCU 2009-10 Dependency Override Request Form
    A student who does not meet the federal criteria for independent status on the Free Application for Federal Student Aid (FAFSA) may submit a request for a dependency status override. The Higher Education Act allows a financial aid administrator to make dependency overrides on a case-by-case basis for students with unusual circumstances.
  • SCU 2009-10 Special Circumstances Form
    When a significant change occurs in parental income due to unemployment, long-term illness, disability or death of a parent.  You will need to contact your Financial Aid Adviser to begin this process.
  • FACHEX Eligibility Certification Form
    This form is required if you are planning on utilizing the FACHEX benefit. Parents can click here to fill out the form and e-mail it directly to the Financial Aid Office.

Federal, State, Other Forms and Applications

  • Free Application for Federal Student Aid (FAFSA)
    All students (incoming students, transfer students and current students) need to complete this form annually.
  • Cal Grant GPA Verification Form
    All California resident undergraduate students who are not currently receiving a Cal Grant need to complete this form
  • CSS/Financial Aid PROFILE
    Incoming undergraduate freshmen and incoming transfer students need to complete this form.
  • 2009-10 Verification Worksheet - Dependent
    If you have been selected for a process called verification for the 2009-10 academic year, through the Quality Assurance Program. You need to submit the worksheet along with appropriate federal tax documents. Please make sure you complete the worksheet and provide all necessary signatures before you submit it to the Financial Aid Office.
  • 2009-10 Verification Worksheet - Independent
    If you have been selected for a process called verification for the 2009-10 academic year, through the Quality Assurance Program. You need to submit the worksheet along with appropriate federal tax documents. Please make sure you complete the worksheet and provide all necessary signatures before you submit it to the Financial Aid Office.
