Ph.D. Checklist

  1. A completed Online Application (includes a 500-word personal statement and the $60 Application Fee paying online)
  2. An official undergraduate transcript
  3. An official graduate transcript (A master's degree is required for all Ph.D. applicants).  For students who are completing their master's degree, send us a transcript in progress.  Upon completion of the degree, it is the students' responsibility to send us a final official transcript, without which students cannot enroll at SCU. **
  4. Three letters of recommendation
  5. Official GRE score report to be sent directly from ETS.  Our institutional code is 4851.
  6. The submission of a TOEFL or IELTS score, sent directly from ETS, is required if English is not your native language or if you do not have a degree awarded by a U.S. university/college. Our institutional code is 4851.

**We accept notarized copies of documents from foreign schools. For documents in English, we accept copies that are certified by a U.S. notary public. For documents in foreign languages, we accept certified translations made by appointed court interpreters in those countries.  International transcripts must be submitted (translated) in English.  Please note: sometimes degree conferral may be issued separately from transcript; if this is the case, the document indicating degree conferral/diploma must also be submitted in English.

Please note:
There are no forms for letters of recommendation, but please include the following:students

  • Name of the applicant must appear in the first line of the text.
  • Must be on company/school official letterhead.
  • Must be a professional reference.
  • Make it as close to one page in length as possible.
