Report a Lost or Stolen Card

Report a lost or stolen card by calling the ACCESS Card Office at 408-551-1647 to immediately to deactivate your card free of charge and limit your liability. After office hours, call Campus Safety Services at 408-554-4441.  DO NOT report a lost or stolen card via E-mail.

If you find your card, reactivate it free of charge at the ACCESS Office.

Replacement cards are issued in the ACCESS Office Mondays and Thursdays from 9:00 am to 6:00 pm and Tuesdays, Wednesdays and Fridays from 9:00 am to 5:00 pm.  

Academic Break office hours are Monday through Thursday, 9:00 am to 1:00 pm and 2:00 pm to 5:00 pm, and Friday, 9:00 am to 1:00 pm and 2:30 pm to 5:00 pm.  

Summer office hours are Monday through Thursday, 9:00 am to 12:30 pm and 1:00 pm to 3:00 pm, and Friday, 9:00 am to 1:00 pm.
