Report a Lost or Stolen Card

If you have lost your ACCESS Card, you can immediately deactivate the card for free and limit your liability by calling the ACCESS Card Office at 408-551-1647 during business hours. After office hours, you may call Campus Safety Services at 408-554-4441 or visit the Online Card Office.  You will need to register at the Online Card Office, if you have not already done so, to suspend your card.  DO NOT report a lost or stolen card via email.

If you find your card, you can reactivate it free of charge at the ACCESS Office.

Replacement cards are issued in the ACCESS Office during normal business hours.

ACCESS Card Office hours during the academic session are Mondays and Thursdays from 9:00 am to 6:00 pm and Tuesdays, Wednesdays and Fridays from 9:00 am to 5:00 pm. 

Academic Intersession Hours are Monday through Friday, 9:00 am to 1:00 pm and 2:00 pm to 5:00 pm. 

Summer office hours are Monday through Thursday, 9:00 am to 12:30 pm and 1:00 pm to 3:00 pm, and Friday, 9:00 am to 1:00 pm.
