Report a Lost or Stolen CardReport a lost or stolen card anytime online at the Online ACCESS Card Office Web site, or call the ACCESS Card Office at 408-551-1647 immediately to deactivate your card free of charge and limit your liability. After office hours, call Campus Safety Services at 408-554-4441. DO NOT report a lost or stolen card via E-mail. If you find your card, reactivate it free of charge at the ACCESS Office. Replacement cards are issued in the ACCESS Office during all office hours, Monday - Friday, 9:00 am - 5:00 pm, with extended hours on Monday & Thursday, 9:00 am - 6:00 pm. Summer and Academic Break hours, Monday - Thursday, 9:00 am - 1:00 pm, 2:00 pm - 5:00 pm, and Fridays, 9:00 am - 1:00 pm, 2:30 - 5:00 pm. |
